Registration and Payment You can download the registration form in PDF format or MS Word format or request your copy by email or phone or write for your copy. To call dial (509) 926-6277 extension 1349 for the Training Class Coordinator; write to the attention of the Training Class Coordinator at Monaco Enterprises, P. O. Box 14129, Spokane, WA 99214-0129. Please complete the registration form and return it by mail or fax it to Monaco as soon as possible to reserve a place in a training class. All classes must be prepaid at time of registration. For immediate registration payment can be made by credit card (MasterCard, Visa, or American Express). Otherwise, payment can be sent by check with the registration form. (Note: American Express requires additional paperwork to process any class registration. Please contact Monaco Enterprises for details) Monaco will send a confirmation letter to the registering student upon confirmation of payment and a completed registration form.
